Name and Address Change Information
All name changes must be made at the Records and Registration office, EAB 105. Address changes can be performed by logging into mavlink (NUID and Password required), or in person at the Registrar's Office.
Changing from Maiden to Married Name
Bring a copy of your new social security card, driver's license, or marriage license to the Registrar's Office and complete the Name Change Form.
Changing from Married to Maiden Name
Bring a copy of your divorce decree, new social security card, or driver's license verifying the name change to the Registrar's Office and complete the Name Change Form.
Legal Name Change
Bring copy of court decree, new social security card,
or
driver's license to the Registrar's office and complete the name change form.
Address Change
Log into mavlink with your student ID and Password or Bring photo ID to the Registrar's Office and complete an Address Change Form. You may also request that your address be changed with a written request mailed to UNO, Registrar's Office, 6001 Dodge, Omaha, Nebraska, 68182. Please note that address changes will not be taken over the phone or via email.
